Does the COVID-19 vaccine cost anything? 

No, the state is providing the vaccine at no cost. 

Do I have to register to get a vaccine? 

Yes. If you are a Northeast Missouri Health Council patient and are interested in receiving a vaccine, you must pre-register for the event ahead of time. 

How do I know if I can get the COVID-19 vaccine? 

As of Friday, April 9, the state of Missouri has activated all phases. This means that all Missouri adults are now eligible to receive the vaccine. 

Will I get my second dose with NMHC as well? 

Yes. If you received the first dose of the Moderna vaccine, we will work with you to schedule a time to get your second dose as well. 

Are there any forms that I need to fill out prior to getting the vaccine? 

We will need one form from you prior to receiving your COVID-19 vaccine. You can download and print it here
