RN-Nurse Care Manager - Kirksville
We’re seeking a RN-Nurse Care Manager to join our team! The qualified candidate will be required to assist in care planning processes, promote disease education and self-management for high risk, chronically ill patients. Certified Diabetes Care and Education Specialist (CDCES) certification is a plus, but can also be willing to obtain certification within the first 2 years of employment. Candidate will participate in patient’s initial treatment plans, healthcare goal development, facilitate team conferences to review treatment goals and discuss utilization of resources, and implement care plans with patients. Candidate will also monitor health information technology (HIT) tools and reports for treatment. Candidate must provide excellent customer service and patient care at all times. Must have good computer skills, electronic medical record (EMR) experience preferred. Qualifications include candidate to preferably have a minimum of two years’ experience in a healthcare setting, but not required.
