Medicaid Insurance

MO HealthNet is Missouri’s Medicaid program. Medicaid covers basic medical and dental services and a variety of other services as well. People who are uninsured and cannot pay for healthcare may be eligible. For any individual who requires assistance with completing their Medicaid application, we have staff who can help.

April 1, 2024.

Family Planning

Keep your information updated with Medicaid

Report any changes throughout the year to the Family Support Division (FSD) as soon as they happen. Changes can be reported on the FSD website (recommended) or in person at your local FSD office. Changes can include:

  • Address changes

  • Change in income

  • Change in number of household members

  • Change in phone number

How to Apply for Medicaid

Have the information you need to apply ready:

  • Your birth certificate (if born outside of the state of Missouri)

  • Missouri Driver’s license (if the person who is applying has had a recent name change)

  • Social Security Card

  • Green Card or Immigration Documents

  • Proof of Income

Not every type of document listed will be requested when applying for just Medicaid; some may be requested for other FSD programs like SNAP or Child Care.  Click here for additional details.

  • What does it cost for my child to participate in the Mobile Dental Program?
    If your child has Missouri care, United Healthcare Community Plan, Home State Health Plan, or Medicaid there is no cost or financial requirement to the parent or guardian. If a child is uninsured, NMHC staff will work with the parent/guardian to verify and/or apply for needed coverage.
  • Does a parent/guardian need to be present when my child is seen on the truck?
    It is not required for a parent /guardian to be present. NMHC arranges for all exams to be completed during school hours to alleviate common scheduling barriers associated with traditional doctor visits.
