Diabetes Care Doesn't Stop After Your First Visit

Managing diabetes is a lifelong journey, but you don't have to do it alone! Whether you've recently been diagnosed or have been living with diabetes for years, ongoing education and nutrition counseling can help you feel more confident, improve your blood sugar levels, and reduce your risk of diabetes-related complications.

At Envision Wellness, we offer two important services to help you reach your health goals: Diabetes Self-Management Education and Support (DSMES) and Medical Nutrition Therapy (MNT).

These services work together to help you develop the knowledge, skills, and healthy habits needed to manage diabetes in your everyday life.

What Is Diabetes Self-Management Education and Support (DSMES)?

DSMES helps you understand diabetes and develop practical skills to manage it successfully.

Through individual appointments and group education, you'll learn how to:

· Monitor and understand your blood sugar levels.

· Use diabetes medications and technology, including continuous glucose monitors (CGMs).

· Recognize and manage high and low blood sugar.

· Make healthy choices involving food, physical activity, sleep, and stress.

· Prevent or delay diabetes-related complications.

· Set realistic goals and overcome everyday challenges.

DSMES is more than learning about diabetes. It's about creating a personalized plan that works for your lifestyle, health needs, and treatment goals. DSMES compliments the care you received from your healthcare provider by helping understand your treatment plan and develop the skills and confidence to put those recommendations into practice. Together, your healthcare provider and diabetes education team help you connect your medical care with the everyday decisions that influence your health.

What Is Medical Nutrition Therapy (MNT)?

Food plays an important role in diabetes management, but there is no single eating plan that works for everyone.

MNT is personalized nutrition care provided by a Registered Dietitian Nutritionist (RDN). Your RDN helps you understand how food affects your blood sugar and develop an eating plan that fits your preferences, lifestyle, and health needs.

During MNT appointments, you may work on:

· Understanding how carbohydrates affect blood sugar.

· Creating balanced meals that fit your food preferences and budget.

· Improving blood sugar, blood pressure, and cholesterol levels.

· Managing weight while meeting your nutritional needs.

· Planning meals around your medications, physical activity, and daily schedule.

· Building healthy eating habits you can maintain long-term.

MNT isn't about following a restrictive diet. It's about finding realistic, enjoyable ways to nourish your body while managing diabetes.

Why Is Completing Follow Ups Important?

Diabetes management isn't something you learn once and then you're finished. Your health, medications, routines, and personal goals can change over time.

At Envision Wellness, we encourage ongoing diabetes education and nutrition follow-up, often every 3–6 months, depending on your individual needs.

Regular appointments give you the opportunity to:

1. Review your progress. Discuss changes in your A1C, blood sugar patterns, and personal health goals.

2. Adjust your plan. Update your education and nutrition strategies as medications, eating habits, or health needs change.

3. Work through challenges. Identify what's working, address barriers, and find practical solutions.

4. Build confidence. Continue developing the skills needed to manage diabetes independently.

5. Celebrate your successes. Recognize improvements and establish achievable goals for the months ahead.

Some people benefit from more frequent appointments, especially when newly diagnosed, starting new medications, experiencing health changes, or having difficulty reaching treatment goals.

Even when your blood sugar is well managed, periodic follow-up can help you maintain your progress and prepare for new challenges.

Specialized Expertise, Personalized Care

At Envision Wellness, our nurses and dietitian work together to provide coordinated, personalized diabetes care. Our team includes advanced expertise in diabetes education, nutrition, and weight management.

Together, our team:

· Reviews your health needs, concerns, and personal goals.

· Develops an individualized diabetes education and nutrition plan.

· Helps you practice self-management skills and make sustainable lifestyle changes.

· Monitors your progress and adjusts recommendations as your needs change.

· Coordinates with your primary care provider to support your overall treatment plan.

For example: If your blood sugar is frequently elevated after meals, our educators can help you identify patterns using your glucose monitor. Your dietitian can also provide individualized nutrition therapy to explore meal composition, carbohydrate portions, and other factors affecting your blood sugar.

This collaborative approach helps ensure that your diabetes education and nutrition care work together rather than as separate pieces of your treatment.

Take the Next Step in Your Diabetes Care

Diabetes education and nutrition therapy aren't just for people who are newly diagnosed. They're valuable resources throughout your diabetes journey. You don’t have to wait until your A1c increases or you’re having difficulty managing diabetes.

Whether you're getting started, working toward a new goal, or simply want to maintain your progress, the Envision Wellness team is here to help.

When was your last diabetes education or nutrition appointment? If it's been more than six months, now may be a great time to reconnect with our team and review your goals.
